Select Contract Notification Form

To register your licenses for the Microsoft Select contract, send the following information. For complicated strings of text, such as the Microsoft Product Number, you might want to use Cut (Ctrl-C) and Paste (Ctrl-V) to avoid any errors.

Filling out this form does not send your order to Software House International or Microsoft. You must generate a PO, LPO or UVa Purchasing Card order separately.
