LSP DCI Service Request Form

This form is for Local Support Partner use when submitting hardware service requests for machines covered by Desktop Computing Initiative (DCI) support. Non-LSP requests or requests for anything other than hardware support will result in the request being rejected and returned to the requestor if possible. Incomplete or invalid contact information may result in the request being discarded entirely if we cannot reach the system's primary user. Requests for service rejected by this process should be called in to the ITC Computing Help Desk at 924-3731.

When asked for UVa Computing ID provide the computing id (e.g.: mst3k) not an email alias or email address.

Problem report must be complete enough to provide to Dell or Apple to justify return. Per Dell's service center site: Please provide a brief description of the problem and troubleshooting performed: ("Bad" and "Dead" are not acceptable descriptions.) NEW: For battery or monitor repairs the problem report must include the serial number on the battery or monitor.
